Planning & Advocacy at Unity Neighborhoods Hub is for people who are tired of repeating their story and still not getting what they need. We help you turn scattered information, loose ideas, and big worries into a clear plan - and then we stand beside you as you speak up for yourself.

We start with Care Plan Development. Together, we look at your whole life, not just one appointment or one diagnosis. We talk through your health needs, daily routines, housing, benefits, family responsibilities, and long‑term hopes. From there, we build a written care plan in plain language that maps out what needs to happen, who is responsible, and when it should be done. This plan becomes your roadmap, something you can share with doctors, schools, caseworkers, or family so everyone is finally on the same page.

Then we provide Advocacy Support to help you use that plan. That can mean preparing you for a meeting, role‑playing tough conversations, or joining you (in person or virtually) when you need backup. We help you explain your needs clearly, ask the right questions, and push for services or accommodations you are entitled to - whether that is more support at home, school modifications, or better coordination between providers.

Because we have been on the other side of the table ourselves, we know how easy it is to feel rushed, dismissed, or talked over. Our role is to make sure your voice is not just heard, but respected.
